Our Office Policies

We have your convenience in mind. With the NEW online patient registration portal you can fill out your patient registration any time of day. Registering online will save time when you arrive for your appointment. This will ensure that you are seen as quickly as possible when you arrive for your appointment. Registration only takes about 10 minutes. Before you start the registration process, have your medical and vision insurance cards at hand. Fill out the information as accurately and completely as possible. You must fill out all required fields for the registration process to be complete. Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.

Emergencies

Our office provides on-call service for emergencies. Call 251-999-0102 so that the physician on call can be contacted.

In case of emergency, please go to your nearest emergency room.  

Payment

The Bay Area Clarity accepts cash and personal checks as well as the following credit cards:

  • MasterCard
  • American Express
  • Discover

We also accept these payment options :

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. We request that we keep a credit card on file, and we will automatically bill any co-pay or deductible to that card on arrival at Bay Area Clarity.  If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.

Patient Forms

The Bay Area Clarity provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
